    We discussed the role of SI investigations/RCA reports in the inquest process, including the interplay between a report prepared to learn lessons but also used as evidence at inquest. We focused on the role then investigation plays as well as how to support staff giving evidence at the Coroner’s Court. Additionally, we explored how these reports, and inquests generally, can impact on the claims process.

    We also touched on the latest draft White Paper on Health and what that could mean for investigations in the future.
